**HALF OF ALL FEES ARE DUE BEFORE SERVICES BEGIN. ALL FEES ARE NON-REFUNDABLE AND DUE WITHIN TWELVE MONTHS OF LICENSING UNLESS OTHERWISE ARRANGED WITH LOVING HOMES, INC

FOSTER ADOPT FEES

UP FRONT FEE OF  $ 2,000.00

These fees include but not limited to the following services:

Application Fee........................................... $150.00
Home Study Fee......................................... $800.00
Agency Fee................................................. $825.50
Pre-Placement Class Fee..............................$175.00
Finger print Fee/FBI Fee when applicable......$39.50
Central Registry Fee$...................................$10.00

Finger print and Central Registry Fees are subject to change at any time from the State or Federal agency.

RELINQUISHMENT OR PRIVATE ADOPTION FEES $ 6,000.00

These fees include but not limited to the following services:

Adoption Fees as stated above..............................$2,000.00
Relinquishment Counseling Fee one parent...........$1,000.00
Counseling Fee two parents additional...................$500.00
Legal Fees (per hour)............................................$175.00
Supervision fee (per month after placement).........$100.00
Mileage (per mile/per month after placement)........$.40
Case management approximately.........................$500.00

In cases of infant adoption from a relinquishment or private placement, the family will bear full financial responsibility for medical expenses incurred by the child from the time of birth to the time of final adoption proceedings when the child is not eligible for Medicaid coverage. The placement family will bear responsibility for birth mother expenses at the time child is placed, with payment made directly to the Hospital

STRAIGHT ADOPTION FROM COUNTY AGENCY  $ 3,800.00

These fees include but not limited to the following services:

Adoption fees as stated above.....................................................$2,000.00
Supervision fee (per month after placement)...............................$100.00
Mileage (per mile/per month after placement).............................$.40
Case management approximately...............................................$500.00

When a child is placed from another state, additional fees will apply to cover the cost of the Interstate Compact: The base rate is an additional.....................................................$1,500.00

**Additional fees may also apply, depending on the complexity of the adoption.

FOSTER CARE ONLY FEES

Central Registry......................................................................$10.00

 EACH PERSON IN THE HOUSEHOLD OVER THE AGE OF 18.

FBI Finger Prints ........................................................................$39.50

 I/We having read the above Fee agreement do hereby agree to pay all fees due to Loving Homes, Inc., and that all fees are non-refundable.
